Fees & Insurance

Fees
Please contact me to discuss payment for services. This will depend on the
type of service, how long a session you would like, and whether you have or
intend to use health insurance. Typical individual and couples therapy
sessions last 45 minutes. Longer sessions are available and often preferred.

Payment
Fees or insurance co-payments are payable by cash, check, Visa, or
Mastercard
at the time of service, unless other arrangements are made.
Under special circumstances, I can establish a billing schedule. To ensure
your full session time, I ask that you pay at the start of each session.

Insurance
The insurance or managed care plans I accept include:

BHS (Behavioral Health Systems)
Blue Shield
CorpHealth
■ Humana
■ Medicare (but not MediCal)
■ MHN (Managed Health Network)
■ Multiplan
■ PHCS (Private Healthcare Systems)
■ Tri-Care/Tri-West
■ EAPs (employee assistance programs)
■ Other pre-arranged managed care payment plans

I will submit appropriate paperwork for reimbursement in these cases.
You are responsible for co-payments and deductibles.

If you are attempting to have your health insurance reimburse you for
part or all of my services, I will be glad to give you an appropriate form
to submit.  Please note that your insurance contract is between you
and your insurance company and the fee remains your responsibility if
for any reason services are not covered. Also note that insurance
companies vary widely in their policies regarding reimbursement for
psychological services and my services may not be covered. I
strongly recommend that you call your insurance company and use
this
checklist (PDF file) to determine your mental health benefits.

I reserve the right to suggest a private pay agreement instead of
insurance reimbursement if your insurance company’s terms require
excessive amounts of my time for obtaining pre-certifications or filling
out paperwork.

Cancellations and Missed Appointments
Since scheduling an appointment means reserving time specifically
for you, I require that you
contact me a minimum of 24 hours (one day)
in advance to reschedule or cancel. If you miss an appointment or do
not cancel within the minimum period, I will have to charge you for
that time at the usual hourly rate.

Out-of-session Services
Telephone consultation longer than ten minutes or out-of-session
services such as writing a letter or report will be charged on a pre-
agreed, pro-rated basis.
